Put the withdrawal form where every customer can find it, typically in your footer or header. Customers fill in their name, email and order number, send it, and get a reference such as WD-7F3K92QX on screen and by email.
Adding it takes a couple of minutes in your theme editor.

The link can use your theme's link style or your own colour, size and underline. The button can use your theme's primary or secondary button, or your own colours and border. You can change the text, for example to match your store's wording, as long as it stays clearly labelled.
In your footer (link or button):
In Withdrawal › Widgets, click Add to footer. Your theme editor opens with the form added to the footer.
Drag it where you want it in the footer, then click Save.
On a page of its own (inline):
In Shopify, go to Online Store › Pages and create a page, for example "Withdraw from contract". Leave the content empty.
In Withdrawal › Widgets, click Add to a page. Your theme editor opens on your page template with the form added.
Click Save. If you don't want the form on every page that uses this template, create a separate page template for it in the theme editor and assign it to your new page.
In Online Store › Navigation, add the page to your footer menu so customers can find it.
By hand: in the theme editor, click Add section, open the Apps tab and choose Withdrawal form. It's a section of its own, so it's listed there and not under Add block inside another section.
